Note to applicants:

Advice for a Smooth Application Process

It is recommended that you copy these questions into a Word document and then answer the questions and edit your answers in that document. When you are ready to submit your project for an award, simply copy and paste the answers from your Word document into the appropriate fields of the on-line application.

This method will allow you to work on your answers until you are satisfied with them at your leisure, and you will have a copy of the questions and answers for your files.

You will be pasting your answers into text only fields. Please be aware of the word limits for the various questions.

No additional documentation beyond what you can paste into the fields will be considered by the judges.

Leadership Award Questions

  1. Please provide the name, title and contact information of the person or each member of the group you are nominating.
  2. Provide specific examples (up to five) of the nominees' demonstrated excellence. Explain in each example why his/her/their work is considered excellent (150 words or less per example).
  3. Provide a brief history of this person's or this group's contribution to Information Technology in your jurisdiction. (300 words or less).


Development Award Questions

  1. Provide a brief project description. (150 words or less)
  2. What business problem were you trying to solve by implementing this project/application? (200 words or less)
  3. Describe the features, functionality and benefits of the project/application. (200 words or less)
  4. Describe the technology used to create the application. (200 words or less)
  5. Did the project/application extend or replace an existing system? If yes, provide a description of what was accomplished. (200 words or less)
  6. How has the business process been improved as a result of the project/application? Provide data that demonstrates this improvement. (200 words or less)
  7. What has been the economic benefit of the project/application (cost savings, cost avoidance, etc.?) (200 words or less)
  8. Who benefits from this use of the project/application? (200 words or less)
  9. What is the current usage of the project/application and what is the population that is eligible to use it? (200 words or less)
  10. How has the project/application been marketed to end-users? (200 words or less)
  11. Provide a link to the project/application if available for external viewing. (200 words or less)
  12. FOR ALL DEVELOPMENT AWARD NOMINEES: Describe the project's collaborative environment in terms of governance, funding, project management, communication and other relevant data. In addition, FOR COLLABORATION AWARD CATEGORY nominees: If you wish to include testimonials or statements of support for your project, please paste this information into your on-line award application.
